REQUEST FOR EXPRESSION OF INTEREST FOR TRANSACTION ADVISER FOR THE EQUIPING OF RADIOLOGY BLOCK THROUGH PPP (PUBLIC PRIVATE PARTNERSHIP) AT NOHE

National Othopeadic Hospital, Enugu is desirous of equipping the Radiology Block through Public Private Partnership (PPP) and therefore request for Expression of Interest (EOI) from reputable companies for Transaction Adviser.

Team Leader must have the following Qualifications
Shall be an expert in Public Private Partnership with certification from ip3 or APMG ii. Have a Project Management Certification of PMP or Prince2
Be a Registered member of his/her professional association for a minimum of ten years.
iv.              Have a current practicing license of his/her profession

Interested companies should submit the Expression of Interest document to Procurement Department with receipt of payment of non refundable fee of N 5,000.00 (Five Thousand Naira Only)

Submission of expression of interest documents closes 12:00 noon Monday, 11th November, 2019 two (2) weeks, The submitted documents will be opened immediately by 12 noon at the New Conference Hall of National Orthopeadic Hospital, Enugu.
Interested bidders, civil society organization and general public are invited to witness the opening of the submission and award on the Monday, 11 th November, 2019 at 12 noon at the new conference hall of National Orthopeadic Hospital, Enugu.
ADDITIONAL INFORMATION:

  1. All documents must be arranged serially as indicated above or the contractor will be disqualified.
  2. Non-compliance to the instruction and late submission will lead to disqualification.

ELIGIBILITY REQUIREMENTS: PROSPECTIVE BIDDERS MUST HAVE THE FOLLOWING:
l. Certificate of Incorporation issued by the Corporate Affairs Commission (CAC)

  1. Certificate of Company's Income Tax Clearance for the last three (3) years valid till 31 st December, 2019;
  2. Certificate of compliance with National Pension Commission Guidelines valid till 31 st December, 2019;
  3. Certificate of compliance with Industrial Training Fund (ITF) guideline valid till 31 st December, 2019;
  4. Nigerian Social Insurance Trust Fund ECS clearance certificate valid till 31 st December, 2019;
  1. Evidence of Registration on the National Database of Federal Contractors, Consultants and Service Providers by submission of Interim Registration Report (RRR) expiring on 31/1/2020 or valid certificate issued by BPP
  2. Sworn Affidavit disclosing whether or not any officer of the relevant committees of the NATIONAL ORTHOPAEDIC HOSPITAL, ENUCJU or the Bureau of Public Procurement is a former or present Director, shareholder or has any pecuniary interest in the bidder and to confirm that all information presented in its bid are true and correct in all particulars; (must be within the period of the advert).
  3. Company's Audited Accounts for the last three (3) years- 2016, 2017, 2018.
  4. Evidence of financial capability to execute the project by submission of Reference Letter from reputable commercial bank in Nigeria, indicating willingness to provide credit facility for the execution of the project when needed; (must be within the period of the advert).
  5. Copy of Memorandum of Association.

l l . Pharmacist Council of Nigeria (PCN) Annual licence of the superintendent pharmacist

  1. Pharmacist Council of Nigeria (PCN) Pharmacy premises registration certificate.
  2. All documents for submission must be ARRANGED SERIALLY as listed above and transmitted with a Covering/Forwarding letter under the Company/.Firm's Letter Head Paper bearing amongst others, the Registration Number (RC) as issued by the Corporate Affairs Commission (CAC), Contact Address, Telephone Number (preferably GSM NO.), and e-mail address. The Letterhead Paper must bear the Names and Nationalities of the Directors of the Company at the bottom of the page, duly signed by the authorized officer of the firm.

METHOD OF APPLICATION

  1. Interested Contractors are advised to obtain Bid Document from the Procurement Unit on presentation of evidence of payment of non-refundable tender fee ofN5, 000.00 (Five Thousand Naira) per lot.
  2. The Technical and Financial documents must be packaged in separate envelopes tagged "Technical Bid" or "Financial Bid" and the Company's name. Technical documents should be arranged as listed above. Both documents should be enclosed in a bigger envelope and addressed to the Medical Director, National

Orthopaedic Hospital, Enugu and inserted into the tender box at the office of the Deputy Director Pharmacy in a sealed envelope not later than 12 noon on Tuesday, 12th November, 2019.

  1. Tenders must be according to the instructions stated above and must be in a sealed envelope marked "TENDER FOR THE SUPPLY OF DRUGS [ITEMS AT THE OFFICE OF THE DEPUTY DIRECTOR PHARMACY OF NATIONAL ORTHOPAEDIC HOSPITAL ENUC,U".

MANAGEMENT IS NOT BOUND TO ACCEPT THE LOWEST TENDER OR ANY.
Issues of note:
( l) The non refundable deposit ofN5,000.00 is per lot

  1. Interested bidder can only bid for the lot(s) he has paid for
  2. Contractors are enjoined to bid for all items in the lot(s) of their choice and stating the sum total for each lot.
  3. Contractors MUST clearly state the lot(s) they wish to bid for together with brand
  4. This advert must not be misconstrued as having awarded contracts to any company.
  5. Management reserves the right to stop the tendering process any time without obligation to the bidders.
  6. Interested Bidders, Civil Societies, Organizations and the General Public are invited to witness the opening ofthe bids and award on Tuesday, 12th November, 2019at 12:00 pm at the Pharmacy Main Hall
  7. Contractors must ensure there is Manufacturers' Authorization for their supplies (where applicable)
